Berberine: A Modern Scientific Interpretation of an Ancient Chinese Medicine
2026-07-02

Berberine is a natural alkaloid extracted from plants such as Coptis chinensis, and has been used in traditional Chinese medicine for centuries. In recent years, it has received widespread attention from the international medical community, primarily due to its unique mechanism of action.

How does it work?

Extensive research points to a key target—the AMPK signaling pathway. AMPK, known as the "cellular energy regulator," is responsible for maintaining the body's energy balance. After berberine activates AMPK, it triggers a series of chain reactions: promoting glucose uptake by cells, improving insulin sensitivity, regulating lipid metabolism, and reducing oxidative stress and inflammation.

In lowering blood sugar, it inhibits hepatic gluconeogenesis (i.e., reducing excessive glucose production in the liver) through the intestinal FXR signaling pathway, thereby lowering blood sugar. In regulating blood lipids, berberine can reduce cholesterol synthesis, promote reverse transport, and improve lipid metabolism by regulating the gut microbiota.

What do clinical studies say?

Multiple randomized, double-blind, placebo-controlled trials have provided direct evidence:

In patients with metabolic-associated fatty liver disease, daily administration of 1500 mg of berberine for 12 weeks significantly reduced alanine aminotransferase (ALT) and total cholesterol levels. Another similar trial found significantly increased vascular endothelial growth factor (VEGF) and decreased systolic blood pressure in the berberine group, with no serious adverse reactions reported.

In a study of patients experiencing weight gain due to antipsychotic medication, the addition of berberine (600 mg/day) for 12 weeks resulted in significantly lower weight, total cholesterol, and LDL cholesterol compared to the placebo group. Furthermore, a large-scale clinical trial (expected to enroll approximately 2000 participants) is currently underway in China to systematically evaluate the effects of berberine in preventing diabetes and cardiovascular disease.

Who should pay attention to berberine? Based on current clinical evidence, berberine may be beneficial for the following populations:

Adults with elevated blood glucose and lipid metabolism indicators

Patients with metabolic-related fatty liver disease

Persons with metabolic syndrome requiring weight management
